In this observational study of frail elders with a recent hospital discharge, 42% experienced post-discharge problems within 2 weeks after discharge, most commonly with follow up appointments and testing. Almost a third of PCPs were unaware of the patients hospitalization, and patients were twice as likely to experience a post-discharge problem if their PCP was unaware of their hospitalization. Better PCP communication systems are absolutely essential to reduce the risk of post-discharge problems (abstract)
