The FDA has issued a warning on the risk of hypomagnesemia with long term PPI use. The proposed mechanism is reduced Mg absorption after long term use. The clinical significance and incidence is not yet known, but some patients in the report did not respond to Mg supplementation and had to be discontinued off their PPI (FDA site)
