In this secondary data analysis of a large clinical trial of >13K patients s/p ACS discharged on clopidogrel or prasugrel, 1/3 of which were on a PPI, those on or off PPI had no difference in clinical outcomes (CV death/MI/CVA). There is an ongoing clinical trial (COGENT-1) in CAD patients on ASA+Clopidogral that will be randomized to PPI or placebo, which should further clarify the clinical relevance of reduced platelet inhibition of PPIs in patients on clopidogrel (abstract)
