New guidelines for non-ICU hyperglycemia

In this joint guideline from the ADA, SHM, and others, major recommendations include target fasting and random blood sugars <140 and <180 respectively, with re-evaluation and modification of therapy for blood sugars <100 and <70 respectively, avoidance of oral agents during hospitalization, and use of scheduled insulin (not sliding scale alone).
