Reported ED medication errors

This analysis of voluntary reporting of medication errors from almost 500 ED’s found most errors occurred at the administration phase, most commonly due to improper dose/quantity, with harm in 3% of them, and communication to the patient/family in <3%. Medication errors in the ED are common, some are harmful, and few are reported to the patient / family (abstract)
