In this follow up of outcomes following medical team training to improve procedural safety in and out of the OR, the rate of adverse events has continued to decrease, from 3.2 to 2.4 a month, with a commiserate increase in close calls from 2 to 3.2 a month. The most common root cause was non-standardization of procedures. This follow up report is a testimony to the efficacy and sustainability of team training in reducing procedural adverse events, in and outside of the OR (abstract)
