For CAP, follow the ATS/IDSA guidelines

In non-ICU patients admitted with CAP, the ATS/ISDA (guidelines) recommend fluoroquinolone monotherapy or combination of cephalosporin / macrolide. In this large retrospective cohort, treatment with either of these was associated with significantly lower mortality (in-hospital, 14 day, and 30 day) (compared to cephalosporin monotherapy). There is no evidence to veer from these simple straightforward guidelines in patients with CAP (citation: Bratzler DW, CID 2008;47:S193)
