Rivaroxaban for post-orthopedic VTE prophylaxis

There have been several RCT with encouraging results using rivaroxaban for VTE prophylaxis after orthopedic surgeries. In this RCT, patients post knee arthoplasty were randomized to oral rivaroxaban 10mg (6-8 hours post op) or SQ enoxaparin30mg q12 (12-24 hours post op) for 10-14 days. The primary outcome (DVT/PE/Death) occurred in 7% of the rivaroxaban group and 10% of the enoxaparin group. Given this is the 4th large RCT indicating superiority of rivaroxaban overenoxaparin in post-orthopedic VTE prevention (RECORD trials 1-4) with similar bleeding risks, rivaroxaban will likely become the treatment of choice for post-orthopedic VTE prevention (abstract)
